Job Details

• Rotating weekends, must be able to do holiday coverage.

Job Requirements

• Driver's License/State ID
• Prime Drug Screening (minimum 10-panel test) within 60 days of start
• Prime Color-blind test (must be completed annually)
• License/Certificate Verification (Primary Source Verification of professional license, and/or certification, as required for the position)
• COVID-19 Vaccine (2 vaccines and booster required or declination completed)
• MMR (Proof of 2 Vaccines or Positive Titer showing immunity)
• Fit Test (Annual Respirator fit test)
• Hepatitis B (Declinations not accepted)
• Influenza Vaccine (Flu Vaccine or declination required October 1-May 31)
• TB Test (Annual TB Questionnaire)
• Varicella (Vaccination/titer showing immunity)
• Tdap (Declinations not accepted)
• Physical/Health Statement (Annually, Except Saint Marys Regional Medical Center- Nevada must be completed within 60 days of start)
• Prime Core Competencies Exam (Must score 80% or higher)
• Prime Criminal Background Check (National Criminal Background Check within one year of start date)
• Prime Badge Photo (JPEG only photo)
• Prime Form I-9 (Employment eligibility verification)
• Prime Job Description (Signed by employee)
• DCI/BVP (Packet found within the document library)
